How Membership Works
After you’ve joined, your benefits begin immediately! You get:
- Two free simple hygiene visits per year (including examinations and beneficial fluoride treatments for all ages)
- All necessary x-rays
- All necessary dental treatment at 20% OFF
- $30 copay for problem focused-visits
- Many services are included that are not covered by most dental insurance, including cosmetic dentistry, occlusal guards for grinding, and anti-snoring appliances!

WHERE ARE SERVICES OBTAINED?
Services for this plan are offered at our office only, located at 6025 Brookvale Lane in Knoxville.
HOW DO I RECEIVE CARE?
After your membership is in effect, simply call our office for an appointment. Our usual office hours: MON, TUES, THURS, FRI; 8:30am – 5:00pm
WHO IS ELIGIBLE?
You, your spouse & any dependent children under the age of 19 or full time students up to age 23 years of age (proof must be provided).
WHEN WILL BENEFITS BEGIN?
Benefit will begin immediately. Members must remain in the plan a minimum of 12 months. This plan cannot be used in addition to another dental plan or dental insurance.
WHAT IS THE ANNUAL MEMBERSHIP FEE?
1 member: $360.00
2 members: $510.00
3 members: $660.00
4 members: $840.00
5 or more members: call us for a quote!
For an individual on our plan, that works out to be $30 per month!

HOW DO I MAKE PAYMENTS FOR SERVICES RENDERED?
Payments are made directly to our office. We also offer financing options for patients who prefer it.
